Swansea Bay UHB Formulary
 
This formulary lists all medicines approved for use in Swansea Bay UHB (for both Primary and Secondary care).
 
Using INFORM
Enter the name of the medicine into the search box located at the top of the screen. This will return search results split into three parts:
  1. Formulary – the drug has already been added to the local formulary.
  2. Non-formulary – the drug is not on the local formulary
  3. Documents – relevant links and documents associated with the drug or BNF chapter
Drugs can also be found via the BNF tree by searching under the relevant BNF chapter and section. Sections of the BNF tree can be expanded using the “+” signs. This method allows you to see alternative formulary options within the BNF chapter/section.

JOINT PRIMARY AND SECONDARY CARE FORMULARY
Under each drug monograph, the formulary status will be colour coded using the following the key:
 
RED (H) - Hospital use only.

AMBER1 (S) - Drug may only be initiated within secondary care (or tertiary care). Prescribing of drug may be transferred to primary care in accordance with formal shared care protocol.
AMBER2 (S) - Drug may only be initiated by (or on the recommendation of) secondary/tertiary care. Prescribing of drug may be transferred to primary care with provision of supporting information e.g. SBUHB Prescribing information sheet, if necessary.

GREEN (1st) – Drug is on formulary and may be initiated and prescribed by primary or secondary care.
BLUE (2nd) – Drug is on formulary and is the 2nd line choice for initiation/prescription in primary or secondary care.
 
SC – Shared care protocol
